Notch just submitted a game to this year's Ludum Dare, a game designing competition where challengers must fabricate a fully functional game in one weekend. This year's theme is "alone". I was hanging out in Notch's stream as he coded the entire thing. Needless to say, he's a pretty fast coder and it's been a pleasure to see how a real pro does it. Access to stairs going up require pickaxes made of ore that can only be found underground. Be sure to bring a lantern. Also, there is no save function.
